Announce the Gift of Sight Contest Winner!
Congratulations to Andi Van Winkle of Davenport, IA she won a FREE LASIK in our Gift of Sight Contest for her "Call 911…my eyes need help!" essay. She was chosen from 200 essays based on number of votes and our judges input. She will receive LASIK surgery with Dr. Richard Phinney, a top 5 LASIK surgeon, in November.
Congratulations also go to Brooke Fischer, who was runner up and will receive 50% off LASIK.

The Gift of Sight Contest
Are you struggling with the hassles of glasses and contacts? Eye Surgeons Associates and Dwyer and Michaels are proud to announce the Gift of Sight Contest. Tell us how LASIK will improve your life, make things easier, what you'll be able to see better, and/or what you'll be able to do with your new vision by writing a short compelling essay. One person will be chosen by Eye Surgeons staff and the community. The winner will receive a free LASIK procedure in November performed by Dr. Phinney to reduce or eliminate their need for glasses or contacts.
The contest begins Monday, September 14th. People residing in the Quad City area who have not had LASIK surgery can submit their story. Stories will be judged on originality, style, and their compelling nature. All entries must be received by 10/02/09. Then the community will have one week to vote for the winner. A winner will be determined and announced by 10/13/09.
You must be 21 years of age to enter and reside within Scott, Clinton, Muscatine, Rock Island, Henry, Whiteside, or Mercer Counties. If the winner is not a good LASIK candidate, the person with the second highest vote score will be chosen. Employees and family members of Eye Surgeons and 97X are not eligible. Please see the official rules.
Medically qualified LASIK candidates can be nearsighted (myopia), farsighted (hyperopia), and/or have an astigmatism. You will have a stable prescription for one year, and a prescription up to +6 for hyperopia or -14 for myopia and no known eye diseases. If you have rheumatoid arthritis, lupus, crohns, ulcerative colitis, scleroderma, uncontrolled diabetes, are pregnant or nursing, wear a pacemaker, or are taking Accutane, Amiodarone, or Imitrex daily, you may not qualify for LASIK.
Official Rules
- No Purchase Necessary. A Purchase Will Not increase Your Chances of Winning. The Gift of Sight Contest ("Contest") is sponsored by Eye Surgeons Associates ("Sponsor") located at 777 Tanglefoot Lane, Bettendorf, IA. No purchase is necessary to enter or win. Odds of winning depend upon the number of eligible and properly qualified entries.
- Contest Period. The Contest Period begins when the site goes live around September 14th and ends at 12:00 pm Friday, October 2nd, 2009. No entries will be accepted before the beginning or after the close of the Contest Period.
- To Enter: During the Contest Period follow the online instructions to submit your entry information, story and release.
An entrant may nominate himself/herself one time. The submission of an Entry shall be deemed to signify the Entrant's acceptance of these official rules. All Entries submitted via the internet must adhere to Eye Surgeons and Glacial's standard terms and condition for user generated content. The Sponsor reserves the right to disqualify any Entry for any reason, and the Sponsor's decision is deemed final in all cases. To be eligible, all Entries must be received during the Contest Period. All Entries become the property of the Sponsor and will not be returned. The Sponsor assumes no responsibility for lost, illegible, incomplete, damaged, late or misdirected Entries, all of which will be void.
- Submission. By entering the Contest and submitting an Entry to the Sponsor, each Entrant represents that: (i) the Entrant is the creator and copyright owner of the story, (ii) the Entrant has granted to the Sponsor an irrevocable license to publish, republish, distribute, create derivatives and copy the edited story an unlimited number of times in any print, electronic or other medium, without additional compensation where permitted by law; (iii) the Entrant has obtained from anyone identified or depicted in the story (or the parent or guardian of any child identified or depicted) all permission(s) necessary to grant the Sponsor the foregoing license; and (iv) the story has not been previously submitted for publication for any other purpose. All Entrants have reviewed the Sponsor's online privacy Policy and agree to the condition of submission stated therein.
- Voting by Users. Eligible Entries properly entered in the Contest during the Contest Period will be displayed online, and all visitors to Eye Surgeons Associates web site (including Entrants and other users) will have an opportunity to vote for their favorite Entry. Viewers of the website will be permitted to vote one time per IP address. Multiple voting by means of software generated or other automated or mechanical processes are prohibited and will be disregarded.
- Selection of the Winners. On or about October 9, 2009 the winner will be selected based on a combination of popular visitor vote and the judgment of a panel of Contest judges, with each part weighted as follows: (A) popular visitor vote (45%) and (B) panel of Contest judges (55%) (the "Standard Contest Weighting"). Contest judges will judge the Entries based on the originality, style, and compelling nature of their essays.
Potential winners will be notified by phone on or about October 13th, 2009 at the phone number designated on the Entry. If Eye Surgeons cannot find an eligible winner for the prize, that prize will not be awarded. All the potential prize winners (the Subjects of the stories) must be evaluated to determine whether he/she is a candidate for LASIK. The winner will be the Subject of the story. Persons who do not qualify for or would not benefit from LASIK are not eligible to accept a prize and if judged ineligible, the prize will be awarded to the next eligible winner. There is no cash prize, no substitution of prizes, and an Entrant or Subject may not designate a third party to receive a prize. The Contest judges' decisions regarding Entries, compliance with the Official Rules, winner eligibility, interpretation of the Official Rules and winners shall be final.
- Prizes. One (1) first place winner will receive the first place prize, which will consist of a bilateral LASIK procedure valued at a maximum of $5,200. The first place winner's winning Entry may also be published on Eye Surgeons Associates web site and in any other publication deemed necessary by the Sponsor. The winner will provide a brief summary of his/her experience before, during, and after receiving LASIK.
First-Runner Up. The first runner up will receive a 50% discount off his/her bilateral LASIK procedure valued at a maximum of $5,200. Surgery must take place by 12/31/09. The Entry of the first runner up may also be published on Eye Surgeons Associates web site and in any other publication deemed necessary by the Sponsor. The first runner up will provide a brief summary of his/her experience before, during, and after receiving LASIK. Neither Sponsor nor any Participating Company shall be obligated to publish or republish any results or summaries of results or photos or stories of Entrants, Subjects or winners. Each winner accepts responsibility for compliance with any and all applicable federal, state and local laws pertaining to use and acceptance of each prize.
- Claiming Prizes. To claim the prize, the winner must respond by email or phone within five (5) business days of prize notification, at the email or phone number designated on the prize notification. Prior to surgery, the winners must: (i) present two (2) forms of proper ID, one of which must be a valid State-issued driver's license or Identification certificate or other such form that will provide proof the Entrant's U.S. residency (e.g. voter registration card, etc.; (ii) sign all required medical releases and other required forms, and (iii) fill out such eligibility declarations, publicity releases, and prize acceptance documentation as requested and provided by the Sponsor. The failure to claim a prize in a timely manner shall result in forfeiture of the prize. All Federal, state and local taxes, if any, are the sole responsibility of the winner. Persons having won a prize valued at $600 or more will be issued an Internal Revenue Form 1099 to report the value of the prize as earned income. Prizes may not be substituted and are not refundable, transferable or redeemable for cash, except at the sole discretion of the Sponsor.
- Eligibility. The Contest is open to legal residents of Scott, Clinton, Muscatine, Rock Island, Henry, Whiteside, and Mercer Counties who are 21 years of age or older. This Contest is subject to all applicable federal, state and local laws and regulations. This offer is void where prohibited or restricted. Employees of Eye Surgeons Associates and 97X, any companies owned by or affiliated with the Sponsor, the Participating Company, the advertising and promotion agencies representing any of Sponsor or its affiliates, and members of such employees' families are not eligible to enter or win. By submitting an entry, each Entrant agrees to all the terms and condition of these Official Rules; agrees to be bound by these Official Rules and the decisions of the Contest judges; and consents to the use of his/her name and other information for purposes of direct mail or similar solicitations where permitted by law. Each winner accepting a prize consents to the use of his/her name and or likeness and any submitted Entry for advertisings, trade and promotional purposes without further compensation, except where prohibited by law. Each Entrant waives the right to assert, as a cost of winning any prize, any and all costs of verification and redemption or travel to redeem a prize and any liability that might arise from redeeming or seeking to redeem a prize. Each Entrant releases each of the following from any and all liability with respect to or arising out of the acceptance, use, procedures, misuses or possession of any prize and/or the loss of misdirection of any prize: Sponsor and the Participating Company; their respective affiliates, subsidiaries, divisions, franchises, advertising and promotional agencies, and their respective officers' directors and employees. Sponsor reserves the right to cancel, amend and/or revoke this Contest any time due to printing errors or other mistakes.
If by reason of a printing or any other error, more prizes are claimed than the number set forth in these Official Rules, all persons making purportedly valid claims will be included in a computerized random selection to award the advertised number of prizes available. No more than the advertised number of prizes will be awarded.
- Gift of Sight. Each winner, by acceptance of the prize, agrees to release all Sponsors, Participating Companies and their parent and subsidiary companies, their affiliates and their officers, directors, employees, agents, shareholders, affiliates, suppliers, distributors, and advertising agencies from all liability, claims, or actions of any kind whatsoever for injuries, damages, or losses to persons and property which may be sustained in connection with the performance, medical follow-up, receipt, ownership or use of the prize. Sponsors and Participating Companies are not responsible for Internet crashes or slowdowns caused by network congestion, viruses, sabotage, satellite failures, phone line failures, electrical outages, natural disasters or acts of man or God. This Contest is not intended for gambling. If it is determined that any Entrant is using the Contest for gambling purposes they will be disqualified and reported to authorities. The Sponsor reserves the right at its sole discretion to cancel, terminate, modify or suspend the Contest if fraud, misconduct or technical failures would destroy the integrity of the Contest. Sponsor reserve the right to change or modify these Official Rules while the Contest is in progress.
- Inquiries. Sponsor does not accept correspondence or telephone inquiries regarding the status of Entries. For a list of winners, send a stamped, self-addressed envelope, accompanied by a signed request to 777 Tanglefoot Lane, Bettendorf, IA 52722 after December 1st, 2009.
